Hours:
Dr. Ventrella sees patients on Mondays and Tuesdays from 1:00pm-5:00pm.
Appointment fees:
- $30/15min – Telemed (Existing Clients Only)
- If you need more than 15 minutes you will be billed an additional $30 for each subsequent 15 minute block (i.e., minute 1-15=$30; minutes 16-30=$30; minutes 31-45=$30)
- For example: A 17 minute appointment will cost $60, a 35 minute appointment will cost $90
- $150 – Follow Up Visit
- $100 – Osteopathic Manipulative Therapy
- $200 – Loomis Digestive Enzyme Therapy
Payment and cancellation policy:
Payment for services is rendered at the time of scheduling the appointment. There will be no exceptions. Refunds will only occur if you cancel a full 48 hours prior to the appointment.
Rescheduling an Appointment:
To reschedule an appointment, please refer back to the confirmation email you received after scheduling your appointment and click the blue “View Appointment Details” button at the bottom of the email.
Forms for appointments:
You will need to download, fill out, and bring these in with you to your Initial Evaluation, these forms must be completed prior to your initial visit. If you are scheduling an Osteopathic Manipulative Therapy, please complete the patient consent form and the patient information form.Follow Up appointments do not require any additional forms.
